Assisting with unplanned and planned moves where the young person has issues which require addressing in order to ensure the move is safe and successful.

May be included in any of our provisions if required.

All referrals should be accompanied by appropriate referral information (including initial assessment, care plan, background information), risk assessment and agreement to funding.
